**Task**
I need you to locate and modify a WordPress quiz plugin, so it meets my needs.
**Problem**
I have tested a quiz program, Modal Survey:

So far I have not been able to make it connect to my autoresponder.
Requirement
I need a quiz plugin.
It should have the following characteristics:
– Likert scale
– Each question is linked to a Category (eg Work or Family)
– Sends the user’s name and email address to my autoresponder, Arigato Pro

Note: I don’t use any of the popular autoresponders, such as Mailchimp, for cost reasons.
