We are looking for upwork talent to design few PDF fillable forms.
The first what we require is a house BL form . If you google the word "sample house BL" you will find 100’s. House BL form what we require is very similar. 1 page the fillable form and the second page the terms and conditions. We will provide the sample for shortlisted applicants.
2nd page is similar to the attachment. You may require to cut and paste the 2nd page but should be in a font where customer can read and must be on one page.
Form must be a professional design form. If you have done similar jobs please send us some samples. We need to complete the job within 24-48 hours.
